NSW: We're responsible but it's not our fault: premier

NSW Premier BOB CARR has pledged to make rail authorities safety fanatics in the wake
of damning findings by the inquiry into the Waterfall train disaster.

The report says the crash -- which killed seven people and injured 42 -- was caused
by the driver suffering a heart attack.

It also says StateRail managers knew of problems with the so-called deadman's emergency
brake for 15 years.

Mr CARR says he'll be seeking advice as to whether individuals will be pursued for
their role in contributing to the derailment.
